How to use it
- Print it when the school’s welcome packet arrives, or any time your child changes schools.
- Sit down with the other adult for 10 minutes and write one name on each “Who” line.
- Open your district’s back-to-school page and add anything it asks for that isn’t here.
- Tape the sheet inside a cabinet door and tick items as they’re done.
- Keep it for next year. Many of these forms come back every fall.

Tips from the paperwork pile
- Start with the forms that have a hard stop. In Washington State, a child needs a Certificate of Immunization Status before attending school or child care.
- Ask for a copy of the vaccine record at the checkup. Doctors keep records of the vaccines they give, and states and some large cities keep a registry too.
- The meal application goes home at the start of each school year, but you can apply any time during the year.

Do I have to fill out the free lunch application every year?
Schools send meal applications home at the start of each school year, and you can apply any time during the year. If your family gets SNAP, your children qualify for free school meals, so ask the school whether you still need a form.
Who should handle the back-to-school paperwork?
Whoever has the time that month, as long as their name is on it. One name per group means the other adult knows which parts are covered.
