A permission slip tracker is one sheet with a row for each form: the event, the due date, the cost, who signed it, when it went back and any notes. Print it, keep it on the fridge, and write each slip down the day it arrives, whether it comes home in the backpack or shows up in a school app.

How to use it
- Print one sheet per child for the school year, and keep it where the backpacks land.
- The day a slip comes home or a school app posts one, write the event and the due date.
- Add the cost and how to pay: cash in an envelope, a check or the school’s payment page.
- Sign it, put your initials under “Signed by”, and put it back in the folder that night.
- Tick “Returned” once it’s back at school, or once the app shows it as submitted.
- Each Sunday, run a finger down the Due column for anything in the next 7 days.

Tips
- Digital slips go on the sheet too. ParentSquare, for example, lets schools send digital permission slips and forms, so one sheet covers the backpack and the phone.
- Aim for the day before the due date. A slip that’s ready Thursday night goes in the backpack Friday morning.
- Keep the money with the slip. Tape the envelope to the form, with your child’s name and the event on the front.
- If two adults sign things, initials show who signed, so nobody signs twice and no slip falls between you.

Questions parents ask
How do I keep track of permission slips?
Write every slip on one sheet the day it arrives, with the due date, the cost and who’s signing it. Tick it off when it goes back, and check the Due column once a week.
What if the permission slip is in ParentSquare or another school app?
Track it the same way. Add a row with the due date, and tick “Returned” when the app shows it as submitted.
What should I do if we lost the permission slip?
Ask the teacher or the front office for another copy or a PDF you can print. Add it to the tracker with the due date so the new copy gets the same attention.
Should both parents sign a permission slip?
Follow what the form asks for. If it has one signature line, one adult signs, and initials on the tracker show which of you did.
When should I send a permission slip back?
As soon as it’s signed, and at least a day before it’s due. If money goes with it, use an envelope labeled with your child’s name and the event.
